Gifts of Securities

Gift of Stocks or Bonds -- In the Name of the Donor
To make gifts of shares of Stocks or Bonds with a Certificate registered in your name the Miller Dwan Foundation will need:

• The Unsigned Stock or Bond Certificate – The Certificate should be sent by Certified Mail to:

• The Stock or Bond Power Form.
  In a separate envelope, mail to the Foundation Office a signed Stock or Bond Power Form – Click here to access the stock or bond power form. For security purposes, never send both the Certificate and Power Form in the same envelope.

You may choose for security reasons to have the Foundation mail the Stock or Bond Power form to you for completion after we have received your unsigned Stock or Bond Certificate. This way you can be assured of secure transmission of both critical pieces. We will provide you with instructions for completing the form and enclose an addressed stamped envelope.

You may at any time bring your Certificate and Stock and Bond Power Form to the Foundation Office in person.

Gift of Stocks or Bonds -- In the Name of Miller Dwan Foundation
To make gifts of shares of Stocks or Bonds with a Certificate registered in the name of the Miller Dwan Foundation, you will need to have your financial advisor or broker register the stocks or bonds in the Foundation’s name.

Please have your financial advisor or broker contact the Miller Dwan Foundation Office at (218) 786-5829 for registration instructions.

Gift from other Brokerage or Trust Accounts
To contribute a gift directly from your brokerage or trust account, it may be done through an electronic security transfer into the Miller Dwan Foundation Trust Account at either Wells Fargo Bank or North Shore Bank.
